pain and malformation after acl replacement

On the last day of ski season, March 30, 2007 (NE Pa.) on my last run i went off the rainbow caught my tip and ripped my ACL in half ,and mcl. I torn some of the middle ring gasket(can't remember the name, sorry). I went to therapy after to rebuild the mcl and had an ACL replacement with paterlar graft in June just as school was out. I am 47 years old and mom of 2 boys who mx and snowboard. I was so disgusted but moved on through therapy. In August i return to mx racing lightly in the women's division.(Motocross). It hurt some but not unbearable. I always wore a brace and didn't push it. When the snow came i returned to skiing, twin tips, because i like the terrain parks , boxes, jumps and rails. My knee is continuing to get worse. It even isn't the same shape as my other knee. When i get home it is swollen , almost looks like fluid. The front part of my knee has a solid swollen lump that is part of my knee. I've had that a while. By time i'm home at night i can't wait t o crawl in bed with an electric blanket and a darvocet with the pain. Well i had the great idea at the young age of 47 to start snowboarding to save on my knees. Been at it for a week and i thing i cracked by tail bone and pelvic bone. I just can't seem to do it. I love to ski and trick and jump but i think it is all over. Should my knee look different than my other. I exercise and always have been very active since i was young. Love dirt bike riding with the boys, racing and skiing. When i came home yesterday, we went from 1 to 8, i was thinking in my head i don't ever want to ski or snowboard again. I was exhausted and in so much pain. I am doing flat boxes, small rainbows, jumps, some 180 and 360's . I try to alway bend to absorb the impact but thing aren't improving. I am so depressed . Why is my knee so big and hard and i still can't bend it all the way like the other. I always walk with a limp. I think 47 is too old for learning snowboarding but is it too old for terrain parks? Any suggestions would be appreciated. Oh, by the way when i was healing after the surgery, the pain was wow! The insicion took forever to heal. It would blister up like a half dollar with pus and bust. The doctor put me on antibiotics acouple of times but it did it for months. I have an ugly scar from it opening all the time. I really don't care but feel like my sporting days are over.

Hi Alyce, I'm really sorry to hear about your knee issues :( - does not sound too good right now. Is the part that you couldn't remember possibly be the medial meniscus?

My son just had back to back ACL replacements, also patella tendon grafts (each knee), second one required meniscus repair/trim as well. He would not have been allowed to do anything like MX that soon after surgery. His total rehab time on each knee was 5 months, and he was a pro slopestyle competitor (snowboard).

Something's not right here, you need to get to your ortho asap. You may have further damaged your meniscus, and the knee needs to be checked for stability to ensure that the graft is still intact. Did you get proprioreceptor retraining following rehab? The knee feels and responds somewhat different following a tendon graft, and it takes time to get the brain and the knee on the same page.

First line of defense here is R-I-C-E, which I'm sure you know what this is, but for any readers who don't: Rest, Ice, Compression, Elevation. Stay off it. Then call the doctor and get it checked. Please.

:booboo: meniscus repair- Yes that was the name. I waited only 3 months to start mx. The pain was tolerable then but the skiing is real bad. Going down steps the pain goes straight under my knee cap too. I don't know what the bone formation is on it. I only had physical therapy not that proprioreceptor one and actually don't know what it is. It looks like knees from 2 different people and swollen at night with fluid or something. I better give him a call. I hate starting over but where i am at isnt where i want to be. I know i am not 13 but do not want to give up sports, yet. My mom is always saying "you are too old for that stuff" but i love going with the boys. thanks alyce

My son didn't have any of the issues you're having, either time, so I'm concerned. What the re-training does is help you become aware of the different way the knee feels and "feeds back" to your brain. A patella tendon is not a ligament - even though it is technically stronger than a ligament, if properly rehabbed. So this training helps you get used to the knee feeling different when it moves.
Get the ice going ASAP.

Yeah, I'd say to go see your doctor and see what they think is going on. Maybe you just need some more physical therapy?

3 months seems to be a really short period of time to get back to doing stuff 100% after an ACL reconstruction. Did you at one point regain the full range of motion in the knee and now have lost it? You also haven't mentioned doing post-op follow-up appointments with your doc, either. Does he or she know about all these things that you just told us?

fwiw, my knees do look different from each other, but I don't really know if this was something that happened b/c of my surgeries or if I started out that way.

Anyway, if more PT is in your future, I hope it includes massage!

I was 46 when I tore my ACL skiing in Telluride. It was tough coming back from the surgery. My knee still looks different than my good knee, even after 5 years. Nine months after surgery, I wa cleared to ski groomers - but only groomers. I stuck to the groomers for the entire season and only at the very end of the season - one year after surgery - did I start going off-piste. It took a lot of patience and a lot of bicycling. I rode my mountain bike or road bike every weekend from the time the doctor said I could ride again until I could ski again. I still do a lot of riding to keep my knee flexible and to keep the muscles and tendons strong. By the way, I also had the graft from my patella. That is no fun! My patellar tendon is still weak. I don't really know any exercises for that other than standing on one leg (the bad leg), holding onto a table top for support and standing on my toes, then standing flat, then standing on my toes, etc... It exercises all the tendons that let your knee cap slide.

There could be any number of things wrong, but one thing to rule out is arthrofibrosis, which is excessive formation of scar tissue, sometimes both inside the joint capsule and outside. It only gets worse with time. It can restrict both bending and straightening and causes lots of pain.

After about six months post-op, if this is the problem, surgery with a specialist is really your only option. I went through a year's nightmare where this wasn't disagnosed and I only got worse. Finally salvaged alot of motion with a second surgery with a surgeon with expertise in treating this. For further good info, go to kneeguru.co.uk
for great info on many knee problems. Esp. look at the soft tissue healing problems/arthrofibrosis area.

Good luck. Don't give up. It took me three years but I'm back on skis and 90% functional.
